An unannounced Renewal inspection was conducted this date. A total of 4 children (11pts) with Provider and 1 assistant present during the inspection. Walk thru of the home was conducted, observation of the programming area, bathroom, kitchen area and crib room was made. Children were observed sitting on the floor with staff singing to nursery rhymes and playing with toys. Observation of hand washing was made prior to lunch. Children transtioned to the lunch table. Lunch today was chicken, pasta, green beans and bananas. Staff observed encouraging the children to eat independently. A total of all 4 children's records were reviewed along with all staff and all household member records. One repeated violation regarding background checks was found along with other violations. Standard #: 22VAC40-111-60-B
Description: Based on child record review, some records were found incomplete. Evidence: Child #3 and #4's record did not contain health insurance policy numbers for each child. Child #4's record did not include the first day of enrollment.

Plan of Correction: I will obtain this information today when the parents arrive.

Standard #: 22VAC40-111-70-A
Description: Based on child record review, some required information provided to parents was not accurate. Evidence: Child #3 and #4's record included a medication policy that indicates that the provider administers prescription medication. The provider does not administer prescription medication and is not trained to administer prescription medication.

Plan of Correction: I will update the medication policy for both these records and have the parents sign.

Standard #: 22VAC40-111-80-C
Description: Based on child record review, required documentation was not found. Evidence: The provider failed to document in Child #2 and #4's record the method of verification for each child's proof of identity.

Plan of Correction: I will obtain a copy of their birth certificates and document the method of verifying.

Standard #: 22VAC40-111-90-C
Description: Based on child record review, updated immunizations were not obtained for some children. Evidence: Child #3 and #4 are under the age of 2, the provider failed to obtain updated documentation of immunizations. Documentation of updated immunizations are required every six months.

Plan of Correction: I will request documentation of the updated immunizations today.

Standard #: 22VAC40-111-170-A
Description: Based on household record review, one TB screening was not obtained. Evidence: Household #2 turned 18 years of age, no TB screening was obtained for this household member.

Plan of Correction: TB screening will be completed this week.

Standard #: 22VAC40-111-180-A
Description: Based on staff and household record review, subsequent TB screenings were not obtained. Evidence: Staff #1 and Household #2's most recent TB screenings were dated 6/2016. Subsequent TB screenings are required every two years.

Plan of Correction: Repeat TB screenings will be obtained this week.

Standard #: 22VAC40-191-40-D-4-a
Description: Based on household record review, background checks were not obtained as required. Evidence: Household member #2 is over the age of 18. No central registry search results have been received to date. No sworn statement has been obtained for Household member #2.

Plan of Correction: The central registry was sent out, the results were not received as yet. I will call today.

Standard #: 22VAC40-191-40-D-5-a
Description: Based on household record review, one household member has not obtained a background check as required. Evidence: Household member #3 is over the age of 14. No central registry search results have been obtained. This remained uncorrected from the February 2018 inspection.

Plan of Correction: The central registry request was sent out, results have not been received as yet. I will call today.
