Provider contacted OBI during this inspection and received her field print code.
An unannounced monitoring inspection was conducted today between the hours of 10:45 am through 12:00. There were a total of 5 children (14 points) with 2 caregivers. A total of 4 children's files and 4 staff/household member's files were reviewed. The children were engaged in the following: toy play, and movie time. Standard #: 22VAC40-111-100-A Description: Based on record review, the provider has not obtained documentation of a child's physical within 30 days after first day of attendance. Evidence: Child # 3, date of enrollment 4/30/18 does not have documentation of a physical examination on file. Plan of Correction: I will ask the parent for the physical.
Standard #: 22VAC40-111-60-B Description: Based on record review, the provider has not maintained a complete record for each child. Evidence: Child #1 and #2 records does not have the medical insurance policy number. (repeat) Child #2 record does not have a 2nd emergency contact name, address and phone number. (repeat) Plan of Correction: I will ask the parents for this information.
