Technical Assistance:
An exit interview was conducted with the provider to discuss the results of today's inspection. Consultation was given regarding updated fingerprint background check regulations.

Comments:
An unannounced monitoring inspection was conducted today, July 27, 2018 to determine the provider's compliance with licensing standards. In attendance were 6 children totaling 14 points being supervised by the provider and assistant. Children were observed interacting with the providers, playing with toys, and eating snack. Children's books, toys, and equipment were inspected and found to be age appropriate and in operable condition. All rooms of the home used by children were inspected with no hazards found. The family day home had the required postings displayed upon entering the home. The first aid kit, fire extinguisher, and emergency supplies were inspected. Three children's records and one provider record were inspected with the majority of the required information found. The following was also inspected: fire drill log, emergency preparedness plan, parent agreements, and other pertinent information. See attached notice for violations found on this day. Violations:
Standard #: 22VAC40-111-830-A
Description: Based on the inspection of administrative records, the family day home has not practiced and recorded the evacuation drills as required. Evidence: During the inspection of the evacuation log, it was discovered the provider did not practice and record the evacuation drills for May and June of 2018.

Plan of Correction: Per provider: We will make sure the evacuation drills are recorded immediately afterward.

Standard #: 22VAC40-111-920
Description: Based on the inspection of administrative records, the food menu was not labeled as required. Evidence: During the inspection of the family day home's snack and lunch menu, it was discovered the menu was not dated.

Plan of Correction: Per provider: I will update the menu with the dates.

Standard #: 22VAC40-111-60-A
Description: Based on the inspection of administrative records, children's records were not up to date.

Evidence:
During the inspection of three children's records, it was discovered child #1 and #2's records were last updated in 2016.

Plan of Correction: Per provider: I will have the parent to review the information and update the children's records.
