Comments:
An unannounced Subsidy Health and Safety (SHSI) inspection was conducted on August 1, 2018, between the hours of approximately 10:50 a.m. and 2:45 p.m. to review the supplemental health and safety requirements. Fourteen (14) children were enrolled and eight children were present, with two caregivers, on this date. Five children's records and five caregiver/household member records were reviewed and/or updated. Program observation was completed. Observed activities included: indoor play (toys); outdoor play; restroom, restroom and handwashing procedures; lunch; infant care and naptime. Adult/child interactions, behavior guidance and daily routines/transitions were also observed. A licensing inspection was also conducted on this date; the two inspections combined covered all of the subsidy health and safety requirements. Only subsidy requirements that were not covered in the licensing standards were reviewed during this supplemental inspection. Violations found during the supplemental subsidy health and safety inspection were identified on the violation notice. Standard #: VENDHOM-000-012-E-1
Description: Based on review/update of four caregiver records and discussion with the provider and assistant #1, on August 1, 2018, one caregiver lacked the demonstration portion of cardiopulmonary resuscitation (CPR) training. Evidence: Review of training documentation and discussion with the provider and assistant #1, on this date, revealed that the demonstration portion of the CPR class was not completed. The CPR course was completed on-line.

Plan of Correction: The demonstration portion of the CPR course will be completed and documentation will be obtained.

Standard #: VENDHOM-000-012-E-2
Description: Based on review/update of four caregiver records and discussion with the provider and assistant #1, on August 1, 2018, one caregiver did not complete the demonstration portion of the first aid course. Evidence: Review of training documentation and discussion with the provider and assistant #1, on this date, revealed that the demonstration portion of the first aid class was not completed. The first aid course was completed on-line.

Plan of Correction: The demonstration portion of the first aid class will be completed and documentation will be obtained.
