22VAC40-191 Background Checks for Child Welfare Agencies
An unannounced renewal inspection was conducted today from 9:30am-11:15am. At the time of inspection, 6 children (16 points) were directly supervised by the provider and one assistant. The sample size consisted of 5 children records, 1 household member record, and 2 caregiver records. Children were observed learning about the daily schedule, engaged in an educational program about letter sounds, and singing songs. Diapering and handwashing procedures were also observed. The children washed their hands prior to eating lunch (rice, beans, apples, and milk). Infants were observed being fed and having floor play with various toys. There was an abundant supply of books, toys, and materials for the children. Repeat violations were found and are identified in this report. Standard #: 22VAC40-111-100-A Description: Based on review of five children's files, the caregiver did not obtain documentation of a physical examination within a child's first date of attendance. Evidence: The record for child #1 (start date: 6/12/2017) did not have documentation of a physical examination. Plan of Correction: The parent brought in updated immunizations and other medical paperwork. The parent will bring in the complete physical.
Standard #: 22VAC40-111-210-A Description: Based on review of two caregiver records, the provider did not ensure all caregivers obtained a minimum of 16 hours of annual training. Evidence: Caregiver #1 had completed a one hour training and caregiver #2 did not complete any training hours. Plan of Correction: In the process of enrolling in a 12 hour course and my assistant will be completing a 10 hour course online.
