Technical Assistance:
Background checks: The number for the Office of Background Investigations is Technical assistance today included the point system, meal planning/preparation, required records. If a parent writes down an allergy, clarify if it is a preference or allergy, have them write that down.

Comments:
An unannounced monitoring inspection was conducted today from 11 am - 1:15 pm. There were 7 children present with the provider and assistant for 17 points. Violations:
Standard #: 22VAC40-111-760-A
Description: Based on a review of first aid supplies today, there was only one triangular bandage present.

Plan of Correction: I will put in another.

Standard #: 22VAC40-111-100-A
Description: Based on a review of records today, there were immunizations but not physical examinations within 30 days for some of the children. Evidence: There was no physical exams for Child 1, 2, 3 and 4.

Plan of Correction: I will talk to their parents and have them bring them in.

Standard #: 22VAC40-111-50-A
Description: Based on observation during today's inspection, attendance was not documented today. Evidence: There was no attendance documented for today. Last week's attendence was posted, but staff confirmed it hadn't been recorded this week.

Plan of Correction: I will write down attendance every day.

Standard #: 22VAC40-111-60-B
Description: Based on a review of children's records today, some information was not complete. Evidence: There was incomplete information regarding parent work address and emergency contact addresses for Child # 1, 2,3 and 4. There was no signature from a parent regarding an annual review and update of emergency contact information for Child # 2, 3, 4. There was no parent signature for the Authorization for Emergency Medical Treatment for Child # 1. There was no health insurance policy number documented for Child # 1.

Plan of Correction: I will get this information as soon as possible.

Standard #: 22VAC40-111-280
Description: Based on observation during today's inspection, a room off the children's area contained hazardous materials and was not made inaccessible. Evidence: The laundry room, where cleaners were located, had a door that was not made inaccessible when children were present. .

Plan of Correction: The child-proof doorknob was placed on the door during the inspection. We will look at other ways to make sure this area in not acessible.
