Comments:
An unannounced monitoring inspection was completed today. The physical plant, 4 caregiver/household member files, 5 children's files, caregiver training, child nutrition, children's activities, emergency procedures, drills and supplies were inspected between 10:50-12:00pm. There were 10 children, totaling 28 points, under the supervision of 3 caregivers. During the inspection, children were observed coloring, learning about colors and shapes, having a diaper changed, being fed (infant) and enjoying free play. The interactions between the children and caregivers were seen to be positive and constructive in nature. Today, the children were served chicken, pasta, mixed vegetables and milk. The Provider is certified in MAT (medication administration training), but there are no children with any medication needs at this time. Standard #: 22VAC40-111-940-B
Description: Based on observations, the Provider did not have an operable thermometer available to monitor refrigerator and freezer compartment temperatures. Evidence 1. The small refrigerator, which contained bottles of milk/formula, did not have a thermometer.

Plan of Correction: The Provider will get one for this.

Standard #: 22VAC40-111-60-B
Description: Based on file review, 2 out of 5 children's records do not have all required information. Evidence 1. Child #1 does not have the addresses for the parent's place of employment. 2. Child #5 does not have the name and policy number of the child's health insurance.

Plan of Correction: The Provider will get this information.
