Technical Assistance:
Discussed with provider background checks, crib sheets and training hours for assistants.

Comments:
An unannounced monitoring inspection was conducted today from 9:50am to 11:20am. There were 10 children (32 points) in care of 3 caregivers. The children were observed being fed, reading books and playing with toys. There were no medications on site. 5 children and staff/household member records were reviewed. Emergency drill records were current. Areas of non compliance are listed in the violation notice. For any questions regarding this inspection please contact me at mahrukh.aziz@dss.virginia.gov.

Violations:
Standard #: 22VAC40-111-960-C
Description: Based on observation, safety waist and crotch straps for high chairs in use with children under the age of 12 months were not fastened. Evidence: 1. Four infants were observed seated and being fed in high chairs. 2. The safety waist and crotch straps were not fastened for any of the four high chairs in use.

Plan of Correction: The assistant fastened the straps during the inspection.

Standard #: 22VAC40-111-100-A
Description: Based on record review, the provider did not obtain documentation of a physical examination for children in care within 30 days after first day of attendance. Evidence: Child # 1's first date of attendance was 3/8/18. There was no documentation in Child # 1's record of a physical examination.

Plan of Correction: I will get the physical from the parent.

Standard #: 22VAC40-191-60-C-2
Description: Based on record review and staff interview, the provider did not obtain a central registry finding for staff within 30 days of employment. Evidence: 1. Staff # 1 was hired on 3/16/18. 2. Staff # 2 stated that she misunderstood that fingerprinting will also include the central registry finding. 3. Staff # 2 did not submit the central registry form for Staff # 1 in the allowable time frame. Staff # 2 stated that she mailed the form in June.

Plan of Correction: I will follow up and check the status of the form.
