An unannounced renewal inspection was completed on August 13, 2018 from approximately 9:40a.m. to 12:45p.m. Ten children were in care, ages two to five years old, with two caregivers, for a total of nineteen points. The children were observed interacting with the caregiver and an assistant, during toileting, hand washing and free play. Children?s toys, learning materials, equipment, bathroom, indoor areas, outside play area and the home?s first aid kit and emergency supplies were inspected. Five children?s records, three staff records, emergency preparedness plan, fire drill and shelter in place logs, daily attendance, injury reports, medication authorizations and logs, medication administration and daily health observation certifications and required postings were reviewed. Transportation is not currently provided. The licensee was available for interview, inspection and present at the exit interview at which time inspection findings were reviewed and an Acknowledgement of Inspection form was signed and left with the licensee. There were five citations for violations of the Standards for Family Day Homes. See the violation notice on the Department's public web site for violations of the Standards.

Standard #: 22VAC40-111-800-B
Description: Based on a review of the emergency preparedness plan, the licensee failed to review the plan at least annually and update the plan as needed. Evidence: 1. The last recorded update of the emergency preparedness plan was observed and dated July 2013. 2. Caregiver #1 stated the plan has been updated annually but acknowledged the dates have not been documented in writing.

Plan of Correction: Licensee will complete the most recent annual update and document the previous updates.

Standard #: 22VAC40-111-90-C
Description: Based on a review of children?s records, the licensee failed to obtain documentation of a child?s additional immunizations once every six months for children under two years of age for one of five children. Evidence: 1. Child #4 (date of birth: May 10, 2016) was enrolled on September 5, 2017. 2. Child #4?s record contained immunization documentation dated August 16, 2017 but lacked any other additional immunization documentation. 3. Caregiver #1 acknowledged this documentation was not obtained.

Plan of Correction: Licensee will obtain updated immunization documentation from parent.

Standard #: 22VAC40-111-230-A
Description: Based on a review of caregiver records, the licensee failed to maintain documentation of each caregiver?s applicable first aid and CPR certifications, and annual training for one of three caregivers. Evidence: 1. Caregiver #1?s record lacked documentation of completed CPR and first aid certifications as well as all completed sixteen hours of annual training. 2. Caregiver #1 stated these certifications have been completed but that the card is not in the record.

Plan of Correction: Licensee will obtain a copy of the missing CPR/First Aid card.

Standard #: 22VAC40-111-320-B
Description: Based on observation of the home, the licensee failed to ensure that child-resistant protective covers are installed on unused electrical outlets accessible to children under five years of age. Evidence: 1. One electrical outlet and one power strip, accessible to children, were observed with no cover on the sockets. 2. Caregiver #1 acknowledged the covers were missing.

Plan of Correction: Licensee removed power strip from use and placed a protective cover on the remaining uncovered socket. Corrected on site.

Standard #: 22VAC40-111-350-E
Description: Based on observation of the home, licensee failed to ensure that hot water taps available to children are maintained within a range of 105 to 120 degrees Fahrenheit. Evidence: 1. Water from the hot water tap in the bathroom accessible to and used by the children was measured at 124 degrees Fahrenheit. 2. Caregiver #1 acknowledged the water exceeded the acceptable range.

Plan of Correction: Licensee adjusted the temperature on the hot water tank. Corrected on site.
