Technical Assistance:
Discussed potential "foods to avoid" and "food intolerances" for children.

Comments:
An unannounced monitoring inspection was initiated today. The physical plant, 3 caregiver/household member files, 5 children's files, caregiver training, child nutrition, children's activities, emergency procedures, drills and supplies were inspected on-site today between 1:20pm and 2:20pm. There were 9 children (plus the Provider's daughter), totaling 20 points, under the supervision of two caregivers. Today, the children played in the ball pit and outside, participated in circle time, listened to story time, ate lunch and rested at nap time. The interactions between the children and caregivers were seen to be positive and constructive in nature. The children were served chicken nuggets, corn, green beans, wheat roll and milk for lunch. The Provider is not currently certified in MAT (medication administration training) and there are no children with any medication needs. Description: Based on observations and interview, the FDH does not have a posted menu for the current one-week period.

Description: Based on file review, 2 out of 5 children's records do not have all required information. Evidence 1. Child #1 and #4 do not have work addresses listed for their fathers.

Description: Based on file review, 1 out of 5 children's records does not have documentation of additional immunizations once every six months up to the age of 2 years. Evidence 1. Child #4 is 28 months old. The most recent immunization record on file was completed when the child was 17 months old.
