Discussed required distance between occupied cribs, and monitoring of each sleeping infant.
An unannounced monitoring inspection was conducted today from 10:30am-1:00pm. There were 9 children (27 points) directly supervised by 3 staff. The home, 3 staff records, 5 children?s records, 4 household member?s records, evacuation drills, emergency supplies, and policies were inspected. There were no medications on site today. Children were observed playing freely, doing crafts, practicing letters by using a cut out of the letter A to make an alligator head, and eating lunch. There was an abundant supply of books, toys, and materials for the children. Areas of non-compliance are identified on the Violation Notice. This violation notice has been revised.

Standard #: 22VAC40-111-100-A Description: Based on review of children's records, 1 of 5 children did not have documentation of a physical examination within 30 days of the child's first day of attendance. Evidence: Child#5's record does not contain documentation of a physical examination, the child's first day of attendance was documented as 6/18/2018. Plan of Correction: We already requested one.
Standard #: 22VAC40-111-120-B Description: Based on record review, the provider did not maintain complete staff records. Evidence: The files of Staff#1 and staff#2 are missing reference documentation. Two reference checks are required before the first day of employment. Plan of Correction: We will get references.
Standard #: 22VAC40-111-50-A Description: Based on record review, there was no documentation of a written record of children's daily attendance. Evidence: A record of today's attendance was available during the inspection, but a record of daily attendance was missing for the week of 8/06/2018 - 8/10/2018, for 8/13/2018 and for 8/14/2018. Plan of Correction: I will write the children's attendance every day.
Standard #: 22VAC40-111-60-B Description: Based on record review, children's records were incomplete. Evidence: Child#2's record is missing 1 of 2 required emergency contacts. Plan of Correction: We will ask them to give me a second emergency contact today.
Standard #: 22VAC40-111-70-A Description: Based on review of children's records, the provider does not have documentation that parents were provided in writing required information before the child's first day of attendance. Evidence: Child#5's record is missing the family day home's policies for the administration of medication. Plan of Correction: We will give them a copy of the form and have them sign.
Standard #: 22VAC40-111-240-A Description: Based on observation, areas and furnishing of the family day home were not maintained in a safe and operable condition. Evidence: In the programming area the plastic lining covering the refrigerator door is peeling off, because the refrigerator is located in an area accessible to children, it could become a chocking hazard. Plan of Correction: We added tape but we'll look for a more permanent solution.
Standard #: 22VAC40-111-540-A Description: Based on observation, it was determined that not all children occupying the feeding table were securely fastened with the safety strap.
Four of five children seating at the infant feeding table were not using the chair's safety straps. The staff member did not secure children as they were placed in the chairs, and she walked away from the table for approximately 1 to 2 minutes to mix paint for an art activity.
Plan of Correction: We will make sure that straps are used at all times.
