54.1-3408 Provider must be MAT certified to administer prescription medication.

An unannounced monitoring visit was conducted today between the hours of 9:12am through 11:15am with the provider on site. There were 5 children (14 points) with the provider. A sample of 5 children's files and 2 staff/household member's files were reviewed. The children were engaged in the following: toy play, coloring and playing with bubbles. Standard #: 22VAC40-111-730-D
Description: Based on observation, medication for caregivers and household members were not kept in a locked place using a safe locking method that prevents access by children. Evidence: 1. A plastic container of medications were on top of the dining room table and were not in a locked place.

Plan of Correction: The medication was moved to a locked place immediately.

Standard #: 22VAC40-111-60-B
Description: Based on record review, three of five children's records were not complete. Evidence: 1. Child #1 was missing one parent's work phone number and names and addresses of previously attended child day care and schools as required by 22 VAC 40-111-80. 2. Child #2 was missing both parent's place of employment, the child's physician's address, one emergency contact address and an additional emergency contact name, address and phone number, child's medical insurance and policy number, names and addresses of previously attended child day care and schools as required by 22 VAC 40-111-80 and immunization record. 3. Child #3 was missing names and addresses of previously attended child day care and schools as required by 22 VAC 40-111-80.

Plan of Correction: I will contact the parents.

Standard #: 22VAC40-111-80-A
Description: Based on record review, the provider did not obtain within seven business days of the child's first day of attendance at the family day home name and location of previous day care programs and schools the child has attended. Evidence: 1. Child #1 (start date 7/30/2018) and Child #3 (start date 12/12/2017) were missing names and addresses of previously attended child day care and schools as required.

Plan of Correction: I will contact the parents.

Standard #: 22VAC40-111-280
Description: Based on observation, potentially poisonous substances such as cleaning supplies were not stored in areas inaccessible to children. Evidence: 1. A container of clorox wipes was under the bathroom sink the children use in a cabinet that was not locked and was accessible to children.

Plan of Correction: I moved the cleaning product immediately and will replace the safety lock that is worn.
