Description: Based on review of 5 children's records, the provider did not have documentation of a physical for a child within 30 days of attendance. Evidence: Child#1's record did not contain a physical examination, the child's first day of attendance was documented as 01/06/2015.

Plan of Correction: We will request documentation of a physical examination.

Standard #: 22VAC40-111-90-C
Description: Based on record review, the provider did not obtain documentation of updated immunizations once ever six months for children under the age of two years. Evidence: Child#1's immunization record was last updated on 10/28/2015. Child#2's immunization record was last updated on 10/30/2017. Child#3's immunization record was last updated on 1/16/2018. Child#5's immunization record was last updated on 7/18/2017.

Plan of Correction: We will request updated immunization records.

Standard #: 22VAC40-111-320-B
Description: Based on observation, not all unused electrical outlets had child- resistant protective covers. Evidence: One unused electrical outlet was observed without a protective cover in the programming area. Four unused electrical outlets were observed without a protective cover in the family room while the children were having movie time.

Plan of Correction: We will cover all unused electrical outlets in all areas used by children.
