Technical Assistance:
Discussed upcoming planned training for provider and assistant in September. Staff training will be reviewed at the next licensing inspection.

An unannounced monitoring inspection was conducted this date. A total of 7 children (19 pts) with Provider and 1 assistant were present during the inspection. Children were observed drawing and coloring with provider and assistant. A nice transition was observed from the drawing activity to floor play with plastic food and kitchen play set. Staff were observed engaging with the children on the floor and encouraging participation. Staff assisted children with putting on their shoes and were taken outside to play. Preparation for lunch, chicken nuggets, green beans and apple sauce was made. Children were observed washing hands and diaper changing procedures were observed prior to lunch. Observation of the crib area was made as well as the diaper changing area and food preparation area. Ample toys, equipment and art supplies were maintained. A total of 4 children's records were reviewed, all staff and household records were reviewed. All postings were found current in the home. No medications at this time and no food allergies. Description: Based on child record review, provider did not complete annual reviews of emergency contact information for two children. Evidence: Child #3's last review of emergency contacts is dated 8/1/18. Child #4's last review of emergency contacts is dated 7/28/2018.

Plan of Correction: I will review the contacts with the parents today.

Standard #: 22VAC40-111-90-A
Description: Based on child record review, documentation of immunizations were not obtained prior to attendance. Evidence: Child #2 began care on 3/6/18. Documentation of immunizations is dated 3/27/18.

Plan of Correction: I will make sure documentation of immunizations are obtained before a child begins care.

Standard #: 22VAC40-111-90-C
Description: Based on child record review, updated immunizations were not obtained for two children. Evidence: Child #2 and #3 is under the age of 2. Documentation of immunizations were not obtained every six months as required.

Plan of Correction: I will request copies of the updated immunizations for both these children.

Standard #: 63.2-1720.1-B-2
Description: Based on review of staff records, one background check was not obtained. Evidence: Staff #2 was hired on 08/20/2018. A fingerprint background check was not obtained prior to hire date.

Plan of Correction: Staff #2 will complete fingerprint background check and submit search results to the licensing inspector within 10 days.
