Today's visit was a renewal inspection. The inspection began at 10:15am and concluded at 1:20pm. There were ten children in care with three caregivers and a total of twenty-five points. During the inspection play time was observed, diapering was observed, infant care was observed, lunch time was observed, nap time was observed, emergency supplies were viewed, all three provider records were viewed and a household members record was viewed, some policies and procedures were reviewed to include the emergency plan, and the outdoor area was viewed. Discussion took place regarding the fall zone requirements surrounding the stationary piece of equipment. There is a trampoline in the backyard the the licensee stated is for family use only. Description: Based on the review of five out twelve children's records, the provider did not have the immunization record for a child in care. Evidence: Child #5 was in care and there was no immunization record documented for the child. Immunization records are required before a child may attend the family day home.

Plan of Correction: The provider contacted the parent the day of the inspection and a copy will be provided the next program day.
