Technical Assistance:
Consulted with provider about background checks, reminded provider to obtain the physical record for child enrolled within 30 days and proof of ID within 7 days.

An unannounced monitoring inspection was conducted today between 10:45am to 12:30pm. There were 10 children (31 points) with 3 caregivers. The children were observed playing with toys, washing hands and using the bathroom, eating lunch and dancing to children's music. The lunch menu included peas, macaroni and cheese, and milk. Standard #: 22VAC40-111-60-B
Description: Based on record review, children's record contain complete information for children enrolled in the family day home. Evidence: 1. Child # 1's record was missing the physician's information, information for two emergency contacts and persons authorized to pick up, first day of attendance, medical insurance information and the provider's emergency plan. 2. Child # 1's record had the medication policy and the liability insurance form, but the forms were not completed and signed by the parent.

Plan of Correction: I will have the aprent complete all the missing information .

Standard #: 22VAC40-111-170-A
Description: Based on record review, provider did not obtain from each caregiver at the time of hire a current result of Tuberculosis (TB) Screening prior to coming into contact with the children. Evidence: 1. Staff # 2 was hired on 8/8/18. 2. Staff # 2s record did not have a copy of the TB screening results in her record. 3. Staff # 1 stated that Staff # 2 ha already done the TB X-Ray and that she will pick up her TB results today.

Plan of Correction: Staff will get the results today.

Standard #: 22VAC40-191-40-D-5-a
Description: Based on staff interview, provider did not obtain a child protective services (CPS) central registry within 30 days of a household member turning 14 years of age. Evidence: 1. Household member #1 reached the age of 14 in June 2018. 2. Staff # 1 stated that she forgot to complete the form and send it out. CPS central registry results were not obtained for household member # 1.

Plan of Correction: I am submitting the form today.

Standard #: 63.2-1720.1-B-2
Description: Based on record review, family day home provider did not obtain fingerprinting results for employee before first day of employment. Evidence: 1. Staff # 2 was hired on 8/8/18. 2. Staff # 1did not obtain the fingerprinting results for Staff # 2 before the first day of employment. 3. Staff # 1 stated that she was not aware that fingerprinting results must be obtained before the first day of employment for new staff. 4. Staff # 1 stated that Staff # 2 was scheduled for fingerprinting today.

Plan of Correction: Staff is going today to get fingerprinting done.
