Technical Assistance:
Consultation was given on obtaining fingerprints by 9/30/2018 as required by the Code of Virginia for all caregivers and adult household members. Fingerprints must be obtained through FieldPrint-the designated vendor for VA DSS. Directions on completing the on-line appointment process can be found on the VA DSS web site under background investigations- Family Day Home providers. Please call OBI at 804-726-7884 to set-up a point of contact or if you have any questions. The Sworn Disclosure Statement and CPS forms are now current for five years and the expiration dates were written on both caregiver forms for the provider.

An unannounced monitoring visit was conducted today from 9:45-11:00. Five children were present with two caregivers for eighteen points which is within ratios for supervision. Two new children's records were reviewed as well as background checks, TB documentation and first aid/CPR certification for both caregivers. The backyard has a play area for children. Am snack today was cereal, bananas and milk. Lunch offered today will be mac and cheese, corn, apple and milk (some parents provide lunch from home). Diapering supplies, cribs and cots were all observed as well as play area on the first floor which is equipped with musical instruments, creative housekeeping area, infant toys, books and dolls.
