- Technical Assistance:
Discussion of forms needed for both staff and children, a mounted gate at the bottom of the stairs once children are enrolled who are not developmental ready to navigate this area successfully, and the discussion of obtaining MAT certification. All areas not determined during this inspection shall be reviewed during the next unannounced inspection.
The following inspection took place between the hours of 10 am through 1:30 pm. Today, I met with the Provider on site at the Family Day Home (FDH) located in Fairfax, Virginia. At the start of this announced initial inspection there were no children. A total of 1 staff file, and 3 household member?s files were reviewed. The FDH home has dedicated their downstairs for the child care area, to include: a large main area, a play room, a nap room, a kitchenette, and bathroom. The FDH also had a changing table, foot operated trash can, and two new cribs (both meeting the new federal regulations). If you should have any future questions, please call or send an e-mail. Standard #: 22VAC40-111-770 Description: Based on observation, the Family Day Home (FDH) does not have a working portable battery-operated weather band (WB) radio with extra batteries. Evidence - 1. On the date of inspection (08/31/2018), a WB radio was not available for review. 2. While an AM/FM radio was on site at the FDH, it did not contain a "weather band" for its specific broadcasting. Plan of Correction: A WB radio shall be purchased for the FDH.
Standard #: 22VAC40-111-790 Description: Based on review, at the time of inspection, the required emergency numbers were not posted. Evidence: 1. On the date of inspection (08/31/2018), the following emergency telephone numbers were not posted: A 911 or local dial number for police, fire, and emergency medical responders; the telephone number for the regional poison control center; and the responsible person for emergency backup care (as required in 22 VAC 40-111-800 A 3). 2. Information was observed by telephone (the poison control number), but it had yet to be posted. Plan of Correction: The regional poison control center number was posted at the time of inspection. All other numbers will be posted.
Standard #: 22VAC40-111-800-A Description: Based on interview, the Family Day Home (FDH) does not have a written Emergency Preparedness and Response Plan (EP&RP). Evidence - 1. On the date of inspection (08/31/2018), an EP&RP was not available for review. 2. The Provider confirmed (during an interview conducted on the date of inspection) that she had yet to complete the EP&RP for the FDH. Plan of Correction: An EP&RP shall be completed and posted for the FDH. This plan shall also be shared with all of the parents.
