Technical Assistance:
Discussed with the provider: Daughter will need to have a CPS check when she turns 14. Also discussed with the provider Calling the OBI office at 804 - 726-7884 to get the information that you need in order for you and your husband to get your finger prints done. They will give you a code and the link to the web site to register and make an appointment to have your fingerprints done. Reminder. There can be no extra blankets, toys or pillows in the crib.

Comments:
An unannounced monitoring inspection was conducted beginning at 2:30 and ending at 3:30. There were two children in carem ages 4 months and 2 years. Children were observed having snack and playing. The baby was given a bottle and the two year old had a snack. Reviewed the files for the two new children. See violation notice. Emergency drills are current and menu is posted. Standard #: 22VAC40-111-100-A
Description: Based on a review of the records of the two children in care, it was determined that one child did not have a physical examination/well baby check . Evidence: Child #2 whose enrollment date is 7/23/18 does not have a physical/well baby check on file.

Plan of Correction: The provider will ask the parent for the most recent well baby check.
