The initial conditional license cannot be issued until the following is forwarded to the Inspector: Central Registry background search findings for the provider and two adult household members; a current TB screening for one adult household member.
An initial inspection for licensure was conducted today. The provider was caring for one child, equaling 3 points. Children's records, administrative documents, caregiver/adult records, background checks, children's nutrition, the physical plant, children's programming, and caregiver training items were inspected today. The provider reported that no children's medications were on site today. The child in care was observed eating snack, playing with toys, assembling puzzles, and interacting with the caregiver. The provider served pasta with tomato sauce and salami, corn, and milk for lunch today. This inspection was completed between approximately 10:00am and 12:40pm. Thank you for meeting with me today. Standard #: 22VAC40-111-760-A-1 Description: Based on a review of emergency supplies, it was determined that the first aid kit is not complete. Evidence: The first aid kit does not include: tweezers, gloves, ice pack, thermometer, triangular bandage, first aid manual. Plan of Correction: I will get a complete first aid kit.
Standard #: 22VAC40-111-770 Description: Based on a review of emergency supplies, it was determined that the provider does not have all required items. Evidence: The provider does not have a flashlight and emergency radio. Plan of Correction: I will get both items.
Standard #: 22VAC40-111-790 Description: Based on observations, it was determined that the provider has not posted all required emergency phone numbers. Evidence: The provider has not posted: the contact information for a back up emergency provider, the poison control phone number; 911. Plan of Correction: I will post the information.
Standard #: 22VAC40-111-800-A Description: Based on a review of emergency supplies, it was determined that the provider has not created an emergency plan. Evidence: The provider does not have an emergency preparedness plan. Plan of Correction: I will create the plan using the model form.
Standard #: 22VAC40-111-920 Description: Based on observations, it was determined that the provider has not posted all required information. Evidence: The provider has not posted the weekly menu with the current one week period dated. Plan of Correction: I will post the menu for each week.
Standard #: 22VAC40-111-940-B Description: Based on observations, it was determined that the refrigerator is not equipped as required. Evidence: The provider does not have a thermometer inside the children's refrigerator. Plan of Correction: I will buy a thermometer.
Standard #: 22VAC40-111-60-B Description: Based on a review of children's records, it was determined that one record was not complete. Evidence: Child record #1 does not include: the address for each parent's place of employment and work phone number; the medical insurance policy number. Plan of Correction: I will ask the parent for the information.
Standard #: 22VAC40-111-240-A Description: Based on observations, it was determined that one item in the child care area has not been maintained safely as required. Evidence: The provider has placed a TV on top of a low shelf in the child care area. This TV is not safely anchored to keep the children from being able to pull the TV down. Plan of Correction: I will anchor the TV to the wall.
Standard #: 22VAC40-111-690-D Description: Based on observations, it was determined that the provider does not clean and sanitize the diaper changing pad as required. Evidence: The provider does not use a soap/water cleaning solution and a sanitizing solution in between diaper changes. Plan of Correction: I've been wiping the diaper pad with a bleach wipe. I will start using a soap/water solution and bleach/water solution.
