Comments:
The licensing inspector conducted an unannounced monitoring inspection on Thursday, September 6, 2018 from approximately 9:22 am to 11:30 am to determine the provider's compliance with licensing standards at the family day home. During this inspection, there were two children present in care totaling four points being supervised by the provider. The licensing inspector observed the children in care participating in a variety of activities to include completing bathroom break, playing with colorful blocks, watching television, dancing while listening to interactive songs, working on words and name recognition and words, and interacting with peers. The first aid kit and emergency supplies, posted emergency contacts, fire, shelter-in-place, lockdown drills were reviewed by the inspector. During this inspection, there were four children's records, one provider's record, and two household member's records were reviewed by the inspector. Standard #: 22VAC40-111-920
Description: Based on observation and interview with the provider, the provider did not ensure a current one week menu was posted . Evidence: During inspection on September 6, 2018, a one week menu was not posted. Interview with the provider confirmed the menu was not posted and the menu available was dated September 4-6, 2018.

Plan of Correction: The provider will complete menu today.

Standard #: 22VAC40-111-140-B-3
Description: Based on review of provider 's record and interview with the provider, the provider did not ensure to maintain a current certification in cardiopulmonary resuscitation (CPR). Evidence: During the inspection conducted on September 6, 2018, there was no documentation found for a current certification in CPR. Interview with the provider confirmed a current certification for CPR had not been received at this time.

Plan of Correction: The provider will complete CPR training by October 1, 2018.

Standard #: 22VAC40-111-140-B-4
Description: Based on review of provider record and interview with the provider, the provider did not ensure to maintain a current certification in first aid. Evidence: During the inspection conducted on September 6, 2018, there was no documentation found for a current certification in first aid. Interview with the provider confirmed a current certification for first aid had not been received at this time.

Plan of Correction: The provider will complete CPR training by October 1, 2018.

Standard #: 22VAC40-111-280
Description: Based on observations, the provider did not ensure all poisonous or hazardous substances are stored in areas inaccessible to children. Evidence: During the inspection on September 6, 2018, in the bathroom area there was one bottle of air freshner with the label "keep out of reach of children" on the sink in the bathroom and this area was accessible to the children.

Plan of Correction: The provider will ensure the bathroom door.
