An unannounced monitoring inspection was conducted today from 10:00am-1:00pm. There were 12 children (29 points) supervised by 4 direct care staff. The home, 4 staff records, 6 children?s records, 3 household member?s records, evacuation drills, emergency supplies, and policies were inspected. There were no medications on site today. Children were observed napping, playing outside, eating snack, engaging in circle time, playing Legos, and completing fingerprint paintings. Handwashing procedures were also observed. There was an abundance of books, toys, and materials, which were found to be developmentally appropriate for the children in care at the time of inspection. Areas of non-compliance are identified in the Violation Notice. Standard #: 22VAC40-111-60-B Description: Based on record review, 2 out of 6 children's records did not contain all of the required information. Evidence: 1. Child #1's (start date: 3/30/2016) record did not contain written authorization of if a caregiver is to administer prescription or nonprescription medication to the child as required by 22 VAC 40-111-700 A 2, and parent acknowledgement of the annual review of emergency contact information. 2. Child #11's (start date: 10/6/2014) record did not contain documentation of dad's work telephone number. Plan of Correction: I will have the parents complete the missing information today.
Standard #: 22VAC40-111-180-A Description: Based on record review, the provider did not obtain a report of tuberculosis screening for each caregiver every two years from the date of the first screening. Evidence: Caregiver D (date of hire: 8/2/2016) most recent TB screening was dated 8/8/2016 (expired: 8/8/2018). Plan of Correction: I will make an appointment for her today.
