An unannounced monitoring inspection was conducted on September 18, 2018 from approximately 1:15 pm to 3:00 pm. Provider and assistant were present with seven children at the onset of inspection, totalling eighteen points. Five children's records were reviewed. Provider and assistant records were also reviewed. Children were observed during nap time.
Standard #: 22VAC40-111-100-A Description: Based on review, provider did not ensure 1 of 5 children's records has a physical examination within 30 days of beginning attendance. Evidence: On September 18, 2018, the record for child #5 (start date August 6, 2018) did not have a physical. Plan of Correction: Child has an appointment for tomorrow. Parent will bring paperwork.
Standard #: 22VAC40-111-90-A Description: Based on review, provider did not ensure 1 of 5 children has documentation of being adequately immunized prior to beginning attendance. Evidence: On September 18, 2018, the record for child #5 (start date August 8, 2018) did not have an immunization record. Plan of Correction: Child has an appointment for tomorrow. Parent will bring paperwork.
Standard #: 22VAC40-111-180-A Description: Based on review, provider did not ensure 1 of 2 caregivers has an updated TB test or screening every 2 years. Evidence: On September 18, 2018, the record for caregiver #1 had a most recent TB test/screening dated August 27, 2016 which is more than 2 years old. Plan of Correction: Caregiver will have the questionnaire/screening completed.
