63.2 Liability Insurance Disclosure

Technical Assistance:
Discussed the importance of maintaining compliance with points and ensuring the health and safety of children in care.

Comments:
An unannounced monitoring inspection was conducted today from 9:30am-11:00am. There were 10 children (24 points) directly supervised by 2 staff. At the start of inspection, the Provider was alone with 10 children (24 points). Prior to departure, an additional caregiver arrived to ensure compliance. The home, 7 children?s records, evacuation drills, and policies were inspected. Children were observed watching television, coloring, building with blocks, playing freely and celebrating a friend's birthday with a snack. There was an abundant supply of books, toys, and materials for the children. An infant was observed being held during bottle feeding. Areas of non-compliance are identified in the Violation Notice. Standard #: 22VAC40-111-750-B
Description: Based on observation, the topical skin product was accessible to children. Evidence: Lotion was being stored in a basket on the sink in the children's bathroom and accessible to children.

Plan of Correction: Topical cream has been stored on a secured location.

Standard #: 22VAC40-111-60-A
Description: Based on review of 8 children's records and interview with the Provider, 1 enrolled child was missing a record. Evidence: Child#1 did not have an enrollment record on site. (start date 8/24/18)

Plan of Correction: Documentation already provided to parent, will follow up with her.

Standard #: 22VAC40-111-60-B
Description: Based on review of 7 children's records, 4 files were missing information. Evidence: 1. Child#2, #3 and #6 were missing documentation of first day of care. 2. Child #4, #5 and #6 were missing identification verification. (start date for Child #4 and #5, 6/2018. Child 6 no start date.) 3. Child#6 was missing documentation of 2 Emergency Contacts name, address and phone numbers.

Plan of Correction: Child records to be reviewed and updated.

Standard #: 22VAC40-111-70-A
Description: Based on review of 7 children's records, 7 records were not provided the following information in writing prior to the child's first day of attendance: Evidence: 1) Child#1 through Child#7 did not have the check in and out procedures. 2) Child#4, #5, and #6 were missing liability insurance disclosure. 3) Child#5 and #6 were missing medication administration policy notification.

Plan of Correction: Child records to be reviewed and updated.

Standard #: 22VAC40-111-250-E
Description: Based on observation, plastic bags that were large enough for a child's head to fit inside were accessible to children under the age of five. Evidence: Plastic grocery bags were stored in an unlocked lower cabinet in the kitchen area accessible to children.

Plan of Correction: We will remove plastic bags to a locked cabinet

Standard #: 22VAC40-111-540-A
Description: Based on observation, the safety strap was not used or securely fastened while a child was in the rocker. Evidence: Child#1 was placed in a rocker without the safety strap being used.

Plan of Correction: We will make sure that children are stap when on rocker.

Standard #: 22VAC40-111-570-A
Description: Based on observation and interview with Provider, she did not determine if an additional caregiver was needed. Evidence: Provider was alone with 10 children (including 2 residential children) ages 4 months to 4 years which totaled 24 points.

Plan of Correction: Provider will have an assistant when the other caregiver cannot be on site.

Standard #: 22VAC40-111-600-B
Description: Based on observation, a child spent more than 30 minutes of consecutive time in an activity saucer and the intervening time period between confinements was not at least one hour. Evidence: Child#1 was in an activity saucer for 35 minutes and then was moved to a bucket seat in a feeding table without play equipment. Licensing Inspector removed Child#1 and placed them on the floor with age appropriate materials.

Plan of Correction: Provider was busy with other kids and inspectors at the same time. Next time will ensure a second provider as needed.
