- Technical Assistance:
Discussed with provider annual trainings and new CPS form.
An unannounced monitoring inspection was conducted from 1:15pm to 2:15pm. There were 6 children (13 points) in care of 1 caregiver. The children were observed arriving from school to the family day home, washing hands, and doing math activities. 6 children records and 3 caregiver/household member records were reviewed. Standard #: 22VAC40-111-60-B Description: Based on record review, the provider did not obtain all information required for children's records. Evidence: Child # 1 and Child # 2's records were missing the information for person(s) authorized to pick up. Plan of Correction: I will get this information from the parents today.
Standard #: 22VAC40-111-180-A Description: Based on record review, the provider did not obtain subsequent Tuberculosis (TB) screening results for adult household members every two years. Evidence: Household Member # 1's most recent TB screening was dated 7/20/16. The TB screening results expired on 7/20/18 and an updated screening was not obtained in two years. Plan of Correction: I will get an updated TB screening for the household member.
