Technical Assistance:
Discussion of updating the Policy for the Administration of Medications form for all children. Blank forms have been given to the FDH Provider upon her completion of the MAT program. In addition, the TB documentation for Staff #1 is set to expire soon. Please remember to update and place new documentation on file.

Comments:
The following inspection took place between the hours of 10 am through 12 pm (noon). Today, I met with the Provider on site at the Family Day Home (FDH) located near Waterford, Virginia. At the start of this unannounced renewal inspection there were a total of 9 children (18 points) with 2 caregivers. A total of 10 children?s files, 2 staff files, and 2 household member?s files were reviewed. The children were engaged in the following: toy play, learning the letter Q and the past tense of simple words, movie time, music, lunch time, bathroom breaks, hand washing, getting ready for nap time, and other organized activities. Standard #: 22VAC40-111-90-C
Description: Based on review, 2 of 10 children's files reviewed indicated that the Family Day Home (FDH) did not obtain documentation of additional immunizations for a child (who was not exempt from the immunization requirements according to subsection B of this section) once every six months for children under the age of two years. Evidence - 1. On the date of inspection (09/19/2018), the immunization documentation for both Child #3, and Child #8 was older than six months. The documentation for Child #3 was dated - 03/13/2018. The documentation for Child #8 was dated - 12/19/2017. 2. On the date of inspection, based on information on file, both children were under two years of age.

Plan of Correction: The parents of each child shall be notified and updated documentation shall be obtained and placed on file.
