Authorization for topical use can be found on the public website under provider forms.

Comments:
An unannounced Renewal inspection was conducted this date. A total of 6 children (23pts) and Provider and 1 assistant was on site during the inspection. When the inspector arrived, children were busy playing with the ample supply of tactile equipment and toys located throughout the programming area. Walk thru of the home was made, and the home was found well organized and supplied with toys and equipment age appropriate for the children currently in care. Additional cribs were added since the initial inspection. All of which meets the CPSC guidelines. Hand washing procedures were observed before and after lunch. Children were rotated from one play area to another to encourage participation. Staff observed assisting infants that are learning to walk to walk with guidance. All 6 children's records and all staff and household member records were reviewed. This inspection began at 10:20am and ended at 11:45am. Thank you for your cooperation today. Standard #: 22VAC40-111-750-A
Description: Based on review of children's records, authorizations for diaper ointments were not obtained. Evidence: Child # 3 and #4 had topical ointments on site. Authorizations were not obtained from the parents.

Plan of Correction: I will obtain these authorizations and maintain them in the children's files.
