22VAC40-191 Background Checks for Child Welfare Agencies
An unannounced monitoring inspection was completed on September 20, 2018 from approximately 10:30a.m. to 12:45p.m. Ten children were in care, ages infant to two years old, for a total of twenty-five points. The children were observed interacting with the provider and an assistant, during outside and inside play time, to include tummy time for the infants, hand washing and lunch and nap times. Children?s toys, learning materials, equipment, bathroom, inside and outside play areas and the home?s first aid kit and emergency supplies were inspected. The provider does not administer medications or provide transportation. Five children?s records, three caregiver records, one household member's record, fire drill and shelter in place logs, daily attendance and required postings were reviewed. There were no injury reports to review. The provider was available for interview, inspection and exit interview at which time inspection findings were reviewed and an Acknowledgement of Inspection form was signed and left with the licensee. There were three citations for violations of the Standards for Family Day Homes. See the violation notice on the Department's public web site for violations of the Standards.
Standard #: 22VAC40-111-780-B Description: Based on a review of children?s records, the licensee failed to review with the parent annually the child?s emergency contact information to ensure the information is current for one of five children. Evidence: 1. Child #4 was enrolled at the family day home on November 30, 2015. 2. Child #4?s record reflected an annual emergency contact review dated for November 2016 and 2018. There was no annual review documented for 2017. Plan of Correction: Provider will ensure that emergency contact information is updated annually.
Standard #: 22VAC40-111-100-A Description: Based on a review of children?s records, the licensee failed to obtain documentation of a physical examination prior to the child?s attendance or within 30 days after the first day of attendance for one of five children. Evidence: 1. Child #1 was enrolled on August 24, 2017. 2. Child #1?s record contained a physical dated October 23, 2017. 3. Caregiver #1 acknowledged the physical was not obtained prior to attendance or within 30 days after the first day of attendance. Plan of Correction: Provider will ensure that physicals are obtained within the required time frames.
Standard #: 22VAC40-111-180-A Description: Based on a review of caregiver records, the licensee failed to ensure that each caregiver and adult household member obtain a tuberculosis screening every two years for one of three caregivers and one of one adult household member. Evidence: 1. Caregiver #1 and Household Member #1?s records contained tuberculosis screenings dated May 13, 2016 and June 10, 2018 and therefore not within the required two year time frame. 2. Caregiver #1 acknowledged these screenings were not obtained timely. Plan of Correction: Provider will ensure that tuberculosis screenings are obtained every two years.
