Licensing inspector discussed mounted gate near stairs and freezer thermometer with the provider.
An unannounced renewal inspection was conducted today from approximately 3pm-5:30pm. There were 9 children (32 points) in the direct care and supervision of 2 caregivers. A sample of 5 children?s records and 2 caregiver's records and 2 household member's records were reviewed. The children were observed eating snack, learning Spanish, playing with blocks, napping and during dismissal. Zero new caregivers have been hired since the last inspection. Standard #: 22VAC40-111-780-B Description: Based on record review, the provider did not review the record of each child with the parent annually. Evidence: The record of Child D was last reviewed 09/20/2017. Plan of Correction: Children's files will be updates by Monday.
Standard #: 22VAC40-111-830-B Description: Based on record review and interview with provider, shelter in place procedures were not practiced a minimum of twice per year. Evidence: 2 shelter in place procedures were not practiced in 2017. The provider stated that one was practiced 04/2017. Plan of Correction: I will do shelter in place drills tomorrow.
Standard #: 22VAC40-111-830-C Description: Based on interview with provider, documentation of shelter in place drills were not maintained. Evidence: The provider stated that a shelter in place drill was conducted April/2017 and April 2018 but were not documented. Plan of Correction: I will do it tomorrow.
Standard #: 22VAC40-111-960-E Description: Based on observation, bottles in the refrigerator were not correctly labeled. Evidence: Two bottle in the refrigerator were not labeled with the date. Plan of Correction: I will label all bottles with the date starting tomorrow.
Standard #: 22VAC40-111-60-B Description: Based on record review, children's records were incomplete. Evidence: 3 of 5 children's records were incomplete: 1) Child A was missing the father's employment information, and medical insurance policy number. 2) Child B was missing medication administration form. 3) Child C was missing 1 of 2 emergency contacts, medical insurance policy number, and medication authorization form. Plan of Correction: Parents will update children's files by Monday.
Standard #: 22VAC40-111-320-G Description: Based on interview with the provider, the chimney was not inspected annually. Evidence: The provider stated that the chimney was not inspected annually. Plan of Correction: I have to make an appointment. I will get it in two weeks.
Standard #: 22VAC40-111-330-A Description: Based on interview with provider, the family day home did not have a landline telephone that did not require electricity to operate. Evidence: The landline telephone available requires electricity to operate. The provider stated that there was not another landline phone in the house. Plan of Correction: I will buy one after I do some research.
Standard #: 22VAC40-80-120-E-2 Description: Based on observation, the most recent violation notice was not posted. Evidence: The last inspection occurred 04/2018. The violation notice posted was conducted 11/30/2016. Plan of Correction: I will post today's inspection.
