An unannounced renewal inspection was conducted today from approximately 11am-1:15pm. There were 3 children (8 points) in the direct care and supervision of 2 caregivers. A sample of 3 children?s records, 2 caregiver's records and 2 household member's records were reviewed. The children were observed napping, playing outside, playing inside and eating lunch. Zero new caregivers have been hired since the last inspection. Areas of non-compliance are identified in this report. The exit interview was conducted with the provider. Description: Based on record review, the provider did not review the children's files with the parent annually. Evidence: The file of Child A was last reviewed 07/2016.

Plan of Correction: Parents will review the file this week.

Standard #: 22VAC40-111-800-B
Description: Based on observation, the emergency preparedness plan was not reviewed annually. Evidence: The emergency preparedness plan was last reviewed 06/2016.

Plan of Correction: I will review and update it today.

Standard #: 22VAC40-111-830-A
Description: Based on record review, emergency evaucation procedures were not practiced monthly. Evidence: The evacuation procedures were not practiced in March 2018 and September 2018.

Plan of Correction: I forgot I will do the drills monthly from now on. I will have a drill today.

Standard #: 22VAC40-111-80-A
Description: Based on records review, the provider did not obtain verification of the identity of the child with in 7 business of the child's first day of attendance. Evidence: There was proof of identity in the file of Child B. The first day of attendance was 08/27/2018.

Plan of Correction: I will ask her mother today and review it within two or three days.

Description: Based on record review, the provider did not obtain a current tuberculosis report from each adult household member. Evidence: Household Member A's tuberculosis exam was last conducted 06/2016.

Plan of Correction: She will complete it as soon as possible.
