Discussed with the provider: Subsidy information - I will email it to you; The baby will need a copy of the well baby check no later than October 17, 2018; Make sure that you post the information that you get from me at each inspection so that parent's can see.;

Comments:
An unannounced inspection was conducted from 10:00 - 11:00. There was one baby in care today. Reviewed paperwork for that child - see violation notice. Home is well set up for the care of infants. Description: Based on a review of the record for the one child in care, it was determined that the record was not complete. Evidence: The record for Child A has only one of two required emergency contacts.

Plan of Correction: The Provider will ask the parent for another emergency contact.
