Provider can download the renewal application from the VA DSS web site and should submit the renewal and fee prior to expiration date of the current license (11/12/2018)
An unannounced renewal study was conducted today from 11:10-12:10. Eight children were present for twenty seven points with two caregivers which is within ratios for supervision. Children had just finished lunch of spaghetti, green peas, fruit and milk. Bottles for very young children are properly labeled. Reviewed two caregiver's records, two house hold member records and two children's records. Emergency preparation for the written emergency plan, supplies, FA/CPR training is current. Both caregivers have the required training hours for the past licensure year. Observed diapering, singing, free play, coloring and infant care. Space has a large playroom, sleeping area on the lower level and a sun room equipped with toys. Both caregivers have Medication Administration Training (MAT) certification.
