An unannounced mandated Renewal inspection was conducted today from 10:30 am to 1:00 pm. There were 4 children (11 points) and 1 caregiver present. Children were observed finishing circle time, dancing, engaging in free play, tumbling, and playing with plastic animals and fruit. Children were also observed washing hands, having lunch (spinach pesto noodles with egg, corn, milk and strawberries). Reviewed children's files, household files, physical plant, emergency plan and drills, menus, attendance and children's play equipment. Standard #: 22VAC40-111-250-C
Description: Based on review of physical plant, items hung from the sides of cribs were not removed when child older than 5 months was present in the crib. Evidence: Child#1, who was older than 5 months, was observed sleeping in the crib with a pacifier holder tied with string hanging on the side railing. This item was removed during the inspection.

Description: Based on record review, 1 of 4 household members, who is 14 years of age or older, did not have Central Registry results completed. Evidence: Household member #4 did not have Central Registry results in their files.

Description: Based on record review, 2 of 3 household members did not have current results of nationwide fingerprint search completed. Evidence: Household member #2 and #3 did not have current fingerprint results in their file.
