A Subsidy Health and Safety inspection was conducted on 10/09/18 to review supplemental health and safety requirements. A review of children and staff/HHM was conduced, a tour of the house was also conducted. A review of the serious injuries report was also discussed. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice. Inspection began: 12:40 pm and ended 3:00 pm approximately
Standard #: VENDHOM-000-012-B Description: Based on review of staff/HHM records the Virginia Pre-Service Training has not been completed within the first 90 days of signing the vendor agreement. Evidence: 1. According to VACMS the vendor signed the agreement on 10/25/16. The assistant has not completed the pre-service training. Plan of Correction: The vendor will assist the assistant with completing the training.
