A Subsidy Health and Safety inspection was conducted on 10/16/18 to review supplemental health and safety requirements. There were no children present during this inspection. Caregiver, household member and a child's record was reviewed. A complete walk through of the family day home was conducted. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice. Description: Based on staff interview, caregiver did not complete the Virginia Pre-Service Training for Child Care Staff within the first 90 days of employment. Evidence: The provider stated that Caregiver # 1's started employment in 2013. Provider stated that Caregiver # 1 did not complete the Pre-service training.

Plan of Correction: I will have caregiver # 1 complete the training.

Description: Based on staff interview, provider did not practice a shelter in place drill twice a year. Evidence: The provider stated that she has the forms for shelter in place drills but she has never practiced a drill.

Plan of Correction: I will practice a shelter in place drill this week.

Description: Based on staff interview, lockdown procedures were not practiced annually. Evidence: Provider stated that she has never practiced a lockdown drill.
