The licensing inspector conducted an unannounced monitoring inspection on 10/16/2018 from 9:34am to 1:28pm. The children were playing with toys, watching television, and most of the children were sleeping upon the inspector?s arrival. Today, there were five children, totaling 16 points, present with two caregivers. Interviews were held with the caregivers throughout the inspection. Additionally, when appropriate the inspector interacted with children. The provider does not administer medication. The provider does not provide transportation. The provider does not have pets. Five children?s records, three caregiver/household member records and one household member's record was reviewed during the inspection. Last emergency drill: 09/18/2018 Last shelter-in-place drill: 09/18/2018 The violations from the previous inspections were checked for corrections. There was one repeat violation found during today?s inspection. Please complete the ?plan of correction? and ?date to be corrected? for each violation listed on the violation notice and return it to me within 5 business days from receipt. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures. Standard #: 22VAC40-111-320-B
Description: Based on observation, the provider did not ensure that child-resistant protective covers were installed on all unused electrical outlets and surge protectors accessible to children under five years of age. Evidence: There were two uncovered surge protector outlets located on the floor within the reach of children. There was an uncovered outlet located next to a crib within the reach of children.

Standard #: 22VAC40-111-600-B
Description: Based on observation, the provider did not ensure that infants and toddlers spend no more than 30 minutes of consecutive time during waking hours, in a confining piece of equipment. The intervening time period between confinements shall be at least one hour. Evidence: The inspector observed two children in an infant/toddler rocking chair for longer that 30 minutes while awake, on two separate occasions.

Plan of Correction: Will make sure children are playing on the floor more.

Description: Based on observation and an interview, the caregiver did not wash their hands with liquid soap and warm running water after diapering. Evidence: The inspector observed the provider diapering four children consecutively without washing hands with liquid soap and warm running water. The provider informed the inspector that they used a wipe to clean their hands.

Plan of Correction: Will make sure to wash hands with every child.

Description: Based on observation, the caregiver did not ensure that children's hands were washed with liquid soap and warm running water after diapering and before eating. Evidence: The inspector observed the caregiver diaper four children and did not wash their hands after diapering. The caregiver did not wash the children's hands before eating.

Plan of Correction: Will make sure to wash children's hands after diapering and before eating.

Standard #: 22VAC40-111-690-D
Description: Based on observations and an interview, the provider did not ensure that the diapering surface was cleaned and sanitized after each use. Evidence: The inspector observed the provider diaper four children consecutively without cleaning and sanitizing the diapering surface after each diaper change. The provider informed the inspector that the diapering surface is sanitized each morning.

Standard #: 22VAC40-191-40-D-5-a
Description: Based on record review, the provider did not ensure to obtain documentation of a central registry check for all household members age 14 years or older. Evidence: The record of household member #4 did not contain documentation of a central registry check.
