Comments:
An unannounced Subsidy Health & Safety inspection was conducted today, September 26, 2018 to determine the family day home's compliance with subsidy requirements. The inspection was initiated at 1:30pm and concluded at approximately 3:45pm. All subsidy requirements were reviewed and overall compliance determined. Administrative records, all areas in the home used by children, and outdoor play areas were inspected. A licensing inspection was conducted on the same date as the SHSI supplemental inspection; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. See attached notice for violations found during this inspection. Description: Based on the inspection of children's records, the required plan was not obtained and available to review.

Evidence:
During the inspection of the five children's records, it was discovered that child #1 with a diagnosed food allergy did not have an allergy plan in the record. Subsidy health and safety requirements stipulate a written allergy care plan for each child with a diagnosed food allergy, to include instructions from a physician regarding the food to which the child is allergic to and steps to be taken in the event of a suspected or confirmed allergic reaction.

Plan of Correction: Per provider: I will inform the parent an allergy plan is needed from the child's doctor.
