Description: Based on the inspection of administrative records, verification of identity was not obtained. Evidence: During the inspection of five children's records, it was discovered child #4 did not have any verification of identity in the record. Licensing standards require proof of identity and age within seven business days of the child's first day of attendance.

Plan of Correction: Per provider: I will obtain the documentation from the parent and place it in the record.

Description: Based on the inspection of the home, a hazardous chemical was found accessible in the bathroom. Evidence: During the inspection of the children's bathroom, a can of aerosol spray labeled "keep out of reach of children" was accessible on the counter top. Licensing standards stipulate that all hazardous materials/chemicals must be inaccessible to children.

Plan of Correction: Corrected on site.
