Discussed timeframes of receiving background search results for new staff.

An unannounced monitoring inspection was conducted this date. A total of 12 children (28pts) with Provider and 2 assistants present during the inspection. When the inspector arrived, the children were in small groups, some completing halloween decorations using handprints and construction paper. Some children were observed free playing with trains, interlocking pieces and looking at books. The younger children were in the smaller area room watching an educational program. Hand washing was observed prior to lunch. Lunch today was chicken, rice, fruit and milk to drink. Staff observed sitting with the children while they ate their lunch. A total of 6 children's records and all staff records were reviewed today. One medication on site, authorization found complete. All topical ointments labeled and authorizations complete. Description: Based on observation of the diapering area, the diaper surface was no longer nonabsorbent. Evidence: The white diaper pad surface had a hole in the surface and is no longer absorbent.

Plan of Correction: The diaper pad will be replaced.

Standard #: 63.2-1720.1-B-2
Description: Based on review of staff records, one background check was not obtained prior to hire. Evidence: Staff #3 was hired on 8/31/18, fingerprint search results were not obtained until 9/10/18.

Plan of Correction: I was not aware that the results needed to be received prior to hire. I will obtain this with any new staff ongoing.
