Discussion was held on the topics of current enrollment, current staffing, new expiration dates of 5 years for all existing background checks, and training hours of 16 hours for each caregiver will need to be obtained before the end of the annual training period.
The following inspection took place between the hours of 8 am through 10 am. Today, I met with the Provider on site at the Family Day Home (FDH) located in the town of Leesburg, Virginia. At the start of this unannounced monitoring inspection there were a total of 7 children (21 points) with 2 caregivers. A total of 9 children?s files, 2 staff files, and 4 household member?s files were reviewed. The children were engaged in the following: drop-off, toy play, music, song, arts & crafts, infant feeding, bathroom breaks, hand washing, movie time, games, imaginative play, infant sleeping, and other organized activities. Standard #: 22VAC40-111-100-A Description: Based on review and interview, 1 of 9 children's files reviewed indicated that the Provider did not obtain documentation of a physical examination by or under the direction of a physician prior to a child's attendance or within 30 days after the first day of attendance. Evidence - 1. On the date of inspection (10/26/2018), documentation of a physical examination for Child #7 was not available for review. 2. The start date for Child #7 was listed as 06/04/2018. 3. The Provider stated (during an interview on the date of inspection) that she believed that this documentation had been obtained, but could not locate it on the date of inspection. Plan of Correction: This documentation shall be found or obtain from the parent of Child #7.
