Technical Assistance:
Discussion of the new subsidy requirements and how they apply to the Family Day Home (FDH). These new requirements became effective on October 17, 2018. It should be noted that the FDH has not yet complied with the following requirement: 22VAC40-665-400. Procedures for the continuity of operations to ensure that essential functions are maintained during an emergency must now be included in the Emergency Preparedness & Response Plan (EP&RP). The facility is to review the new requirements (sent to the FDH by e-mail) and work with their assigned inspector to ensure future compliance. If this requirement is not found to be in compliance after December 14, 2018, it shall be documented on a violation notice issued to the FDH.

A Subsidy Health and Safety Inspection (SHSI) was conducted at the DeGallo FDH located in Ashburn, Virginia. This inspection was conducted on 10/30/2018, between the hours of 11 am through 2:30 pm to review supplemental health and safety requirements. An Unannounced Monitoring Inspection was conducted on the same date as the SHSI supplemental inspection; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. No violations of the supplemental subsidy health and safety requirements were found. If you should have any future questions, please call or send an e-mail. Thank you. Charlie Perkins, Licensing Inspector (703) 309-3963

A compliance history is in no way a rating for a facility.

The online compliance history includes only information after July 1, 2003. In addition, the online compliance history includes information regarding adverse actions that may be the subject of a pending appeal. An adverse action is not final until a provider has exhausted or waived all due process rights. For compliance history prior to July 1, 2003, or information regarding the status of pending adverse actions, please contact the Licensing Inspector listed in the facility's information. The Virginia Department of Social Services (VDSS) is not responsible for any errors in or omissions from the compliance history information.
