12 children were in care during the renewal inspection. The provider and two assistants were in care. The children were observed during free play and snack time. No children were on medications. Inspection time 1:00pm-3:00pm

Description: Based on review of two house hold member's records and interview with the provider not all had a current T.B. test with in the past two years. Evidence: House hold member A's T.B. test is dated 3/26/2016.

Plan of Correction: They will take one when back in town.

Standard #: 22VAC40-111-210-A
Description: Based on review and interview with the provider not all staff competed 16 hours of annual training. Evidence: Staff A and staff C did not compete any training hours and staff B completed six hours of training.

Plan of Correction: We will complete the required hours and the missed hours from this year.
