An unannounced monitoring inspection was conducted today. The provider and one assistant caregiver were caring for 5 children, totaling 13 points. Children's records, background checks, caregiver records, emergency preparedness items, the physical plant, and children's nutrition were inspected today. The provider reported that no children's medications were on site today. The children were observed finishing lunch, listening to stories, and preparing for their afternoon nap. The children were served lentils, rice, squash, bananas, and milk for lunch today. This inspection was completed on site between approximately 11:15am and 12:15. Standard #: 22VAC40-111-750-A Description: Based on a review of children's topical ointment products, it was determined that one item was not labeled as required. Evidence: One child's non-prescription topical diaper ointment product was not labeled with the child's name. Plan of Correction: The ointment was immediately labeled during the inspection.
Standard #: 22VAC40-111-120-B Description: Based on a review of caregiver records, it was determined that one record was not complete. Evidence: The record for assistant caregiver A does not include emergency contact information. Plan of Correction: I will ask her to complete the form today.
Standard #: 22VAC40-111-60-B Description: Based on a review of children's records, it was determined that one record is not complete. Evidence: Child record A does not include the last name for one emergency contact person. Plan of Correction: I will ask the parent for the missing information.
Standard #: 22VAC40-111-280 Description: Based on observations, it was determined that three potentially poisonous materials were not stored in a manner to keep them inaccessible to children. Evidence: Three bottles of cleaning products were stored inside a cabinet beneath the children's handwashing sink in the bathroom. The door to this cabinet was not locked. These items were stored at a level that made them accessible to the children. Plan of Correction: During the inspection, the cleaners were immediately moved to a wall shelf.
Standard #: 22VAC40-111-690-D Description: Based on observations and caregiver interviews, it was determined that the diapering surface was not cleaned as required in between each use for diaper changing. Evidence: There was no prepared soap/water solution available for use at the diaper changing station. The provider indicated that diaper changes had taken place earlier in the morning, without the use of soap/water. The diaper changing pad was not cleaned with a soap/water solution in between each diaper change, as required. Plan of Correction: I will make sure the soap/water solution is prepared for children arrive each morning.
