Please reminder to practice "Lock down" drill once a year.
An unannounced Subsidy Health and Safety Inspection (SHSI) was conducted today. There were no children in care at the time of the inspection. The children attend after school. Previous violations have been addressed. Areas of non-compliance are identified in this report. The exit interview was conducted with the vendor/provider. Inspection conducted 12:20 pm to 1:40 pm. Standard #: VENDHOM-000-005-B Description: Based upon record review and interview, the vendor did not maintain a written hard copy record of daily attendance that documents the arrival and departure of each child in care as it occurs. Evidence: The vendor stated that she had not kept a written attendance when children arrive and leave each day . Plan of Correction: I will start doing a written attendance today when the kids come from school.
Standard #: VENDHOM-000-006-B Description: Based upon record review, required child information was found to be missing or incomplete. Evidence: Child A's information emergency contact address, authorization for medical treatment Child B's information emergency contact address, authorization for medical treatment Child c's information emergency contact address Plan of Correction: I will make copies and have a form for each child in care.
Standard #: VENDHOM-000-013-C-2 Description: Based upon observation, Electrical outlets did not have protective covers that are of a size that cannot be swallowed by children. Evidence: In the dining area there was one outlet that did not have an outlet cover as required. Plan of Correction: I will put cover in outlet.
Standard #: VENDHOM-000-029-A Description: Based upon record review, the family day home did not have a written emergency preparedness plan that addresses caregiver responsibility and home readiness with respect to lockdown procedures. Evidence: The family day home vendor did not have the required "lock down" senario added to the existing Emergency Response Plan. Plan of Correction: I will get new form and add Lock down to it.
