Standard #: 22VAC40-111-100-A Description: Based on a review of children's health records, it was determined that the provider has not obtained documentation of one child's physical exam within 30 days of the child's date of enrollment. Evidence: Child E entered care more than 30 days ago. There is no physical exam report in this child's record. Plan of Correction: I will remind the parent to bring the physical form completed and signed by the doctor.
Standard #: 22VAC40-111-110-A Description: Based on a review of children's health documents, it was determined that one child's immunization record has not been correctly completed on the current VA Department of Health form. Evidence: The immunization record for child E does not include the physician's signature indicating that the child has been adequately immunized for his/her age. Plan of Correction: I will remind the parent to have the statement signed on the form.
Standard #: 22VAC40-111-60-B Description: Based on a review of children's records, it was determined that two records are not complete. Evidence: Child record C does not include the last name for one person listed an emergency contact. Child record E does not include documentation showing that the parent has reviewed the child's contact information within the past year. Plan of Correction: Both parents will be asked for the information today.
Standard #: 22VAC40-111-90-A Description: Based on a review of children's health records, it was determined that one record does not include an immunization record as required. Evidence: Child D entered care on 10/29/18. This child's record does not include a shot record. The provider did not obtain documentation of a current immunization record before the child entered care. Plan of Correction: I thought the shot record was required within 30 days. I will tell the parent today to bring me the child's shot record.
