Technical Assistance:
Discussed the fingerprinting process and letters of eligibility.

Comments:
An unannounced monitoring inspection was completed today. The physical plant, 5 caregiver/household member files, 5 children's files, caregiver training, child nutrition, children's activities, emergency procedures, drills and supplies were inspected between 1:50-3:00pm. There were 7 children, totaling 16 points, under the supervision of the 2 caregivers. Today, the children had circle time, played with puzzles, colors leaves and pumpkins, and sang songs. The interactions between the children and Provider were seen to be positive and constructive in nature. Today, the children were served chicken nuggets, spaghetti, broccoli, carrots, bananas and milk. The Provider is not certified in MAT (medication administration training), and there are no children with any medication needs. Standard #: 22VAC40-111-60-A
Description: Based on file review, 2 out of 5 children's records have not been updated annually. Evidence 1. Children #4 and #5 have records which were last updated in October 2017.

Plan of Correction: The Provider will ask the parents to update their child's file.

Standard #: 22VAC40-111-60-B
Description: Based on file review, 4 out of 5 children's records do not have all required information. Evidence 1. Child #1 does not have the policy number for their health insurance. 2. Child #5 does not have 2 emergency contacts in case the parents cannot be reached.

Plan of Correction: The Provider will ask the parents for this information.

Standard #: 22VAC40-111-90-A
Description: Based on file review, 1 out of 5 children's records does not have an immunization record prior to the child attending the FDH. Evidence 1. Child #2 was in care today as a drop-in, due to school being closed today. There was no immunization on file.

Plan of Correction: The Provider will ask the parent to bring this to the FDH the next time the child comes here.
