Discussed with the provider: Assuring that at least one of the two required emergency contacts is local. Also discussed assuring that outlets on the floor where the children arrive are covered.
An unannounced monitoring inspection was conducted beginning at 10:45 and ending at 12:10. There were 7 children in care with the provider and an assistant. The children were ages 8 months through 3 years. One of the children lives is the home. Children were observed playing with age appropriate toys and interacting with the providers. Children were also observed having lunch. Reviewed files for 6 children - see Violation notice. Reviewed files for provider and family - complete Emergency drills are up to date. Standard #: 22VAC40-111-100-A Description: Based on a review of the records for 6 children, it was determi.ned that 3 children did not have physical examination (well baby check) on file Evidence: Child #1 Date of enrollment 3/28/18 - no well baby check; Child #2 Date of Enrollment 2/21/18 - no Well Baby check' Child #3 - Date of Enrollment 8/21/17 - no Well baby check. Plan of Correction: All children will have physical examination/well baby check within 30 days of enrollment
