1/08/2018 Arrived 2:00pm Departed 3:40pm An unannounced monitoring inspection was conducted today. Eight children were in care for a total of 16 points with one caregiver. The children were eating a snack, watching TV and resting. Five children's files and two staff files were reviewed. Thank you for your cooperation. Description: Based on the review of staff files and an interview of the provider, the facility failed to insure that all staff have current first and CPR. Evidence: 1. The first card for the assistant expired in March 2018 and the card for the provider expired in October 2018.

Plan of Correction: Both caregivers will enroll in a first aid and CPR class.

Description: Based on a review of documents and an interview with the provider, the facility failed to insure that all staff have current TB test results on file. Evidence: 1. The TB test results on file are dated 10/2016.

Plan of Correction: The caregiver reported that they had an appointment on 11/4/18 and forgot it. Another appointment will be scheduled for 11/19/18.
