Today was a follow-up inspection to the initial inspection originally conducted on August 31, 2018. Discussion was held on the topics of current enrollment, current staffing, care of child in care; and a review of training hours, menu, and posted information.
The following inspection took place between the hours of 10 am through 11:30 am. Today, I met with the Provider on site at the Family Day Home (FDH) located in Fairfax, Virginia. At the start of this unannounced non-mandated monitoring inspection there was 1 child (4 points) with 1 caregiver. A total of 1 child?s files, 1 staff file, and 3 household member?s files were reviewed. The child was engaged in the following: infant feeding, nap time, music, song, and other nurturing activities. Standard #: 22VAC40-111-60-B Description: Based on review, the 1 child's file in review did not contain all of the required information/documentation. Evidence - 1. On the date of inspection (11/09/2018), while the Family Day Home's Policy for the Administration of Medication form was filled out, it was not signed by the parent of Child #1 and the Provider. 2. The start date for Child #1 was listed as 10/09/2018. Plan of Correction: This form will be reviewed, signed by all parties, and placed on file.
