63.2 Licensure and Registration Procedures
An unannounced monitoring inspection was conducted from 11:15 am - 1:00 pm with the provider and her assistant. There were nine children in care, ranging from 16 months to five years of age, for a total of 19 points. The children were observed having free play with a variety of toys including legos, dress up clothes and pretend kitchen food, and preparing for and eating lunch. Lunch served today: ham and cheese sandwiches on whole wheat bread, broccoli, sliced apples and milk. Four children's records were reviewed and in compliance. Two caregiver records were reviewed. The emergency drill log was reviewed. The provider is not Medication Administration Training (MAT) certified and cannot administer medications to children in care. Standard #: 22VAC40-111-830-A Description: Based on observation of the emergency drill log, it was determined that the provider failed to conduct a monthly fire drills as required. Evidence: The last documented fire drill was conducted on 9/28/2018. Plan of Correction: Will conduct a practice fire drill each month.
Standard #: 22VAC40-191-60-B Description: Based on review of two caregiver records, and an interview with the provider, it was determined that not all caregivers had a completed sworn statement or affirmation before employment. Evidence: Caregiver B (date of hire 11/5/2018) did not have documentation of a sworn statement in their file. Plan of Correction: Will have the caregiver sign a sworn statement today.
Standard #: 63.2-1721.1-B-2 Description: Based on review of two caregiver records, and an interview with the provider, it was determined that not all caregivers had an Office of Background Investigations (OBI) fingerprinting-based criminal history record determination letter prior to a beginning employment. Evidence: Caregiver B's (date of hire: 11/5/2018) record did not include an OBI fingerprinting determination letter. Plan of Correction: Will have the caregiver obtain a fingerprint-based background check immediately.
